After discharge from Grantham and District Hospital
NHS reablement may be in place when someone comes home. It is short-term, usually up to six weeks, and aims to rebuild independence. When it ends, ongoing support has to be arranged separately.

Most Grantham families fund care in one of three ways:
- Self-funded. You pay the full rate and have full choice of provider.
- Local Authority Direct Payment. Administered by Lincolnshire County Council for Grantham, and by Leicestershire County Council for Bottesford. The council decides eligibility and the rate. It is usually below our private rate, so some families top up the difference.

Bottesford is in Leicestershire, where the council calls it a care and support assessment. First Contact Plus is its information and advice front door. Call 0116 305 0004, Monday to Thursday 8.30am to 5pm, and Friday until 4.30pm. The Emergency Duty Team is 0116 305 0888.
Leicestershire’s own guidance says there is a waiting list, with priority given to the people who need help most. Some families arrange care privately while they wait. Local authority funding covers the council route in more detail.
